Role Summary 

This position oversees all aspects of The WellHouse Next Step to Independence Case Management, 50% of work time will be after regular business hours to include nights and weekends. 

Responsibilities

  • Provides a continuum of Case Management services to WellHouse residents in the Next Step to Independence program
  • Guides the development and implementation of long-term goals and objectives aimed at achieving a successful NSI experience based on resident input
  • Meets regularly with each NSI resident to establish and monitor an individualized case plan with goals that are designed to lead to successful independent living
  • Refers resident clients to appropriate services including but not limited to: medical, social, educational and employment
  • Maintains data entry of all case management activities
  • Regularly works with resident clients to overcome obstacles that hinder progress
  • Enacts appropriate reporting and disciplinary measures if resident fails to comply with NSI policy expectations
  • Meets weekly with other social services staff to strategize on issues related to residents and how to best assist them in making progress.
  • Ensures that case management and program activities operate within TWH policies and procedures
  • Ensures that program activities comply with all relevant legislation and professional standards. Reports concerns to the Program Manager
  • Works with all administrative staff to ensure the effective and efficient program delivery
  • Coordinates the delivery of services among different program activities to increase success, effectiveness and efficiency.
  • Ensures that calendar activities and appointments are scheduled and on their perspective calendars
  • Perform other duties as assigned by Program Manager, Chief Operations Officer, Executive Director and/or Board

Qualifications

  • Character references
  • At least 25 years of age
  • Excellent communication and organizational skills
  • Excellent interpersonal skills
  • Must be able to drive a large van
  • Adherence to high ethical standards both on and off campus that doesn't hinder the mission of The Wellhouse
  • Must be able to be insured by the ministry's risk management insurance
  • Must be able to successfully prepare meals for groups of ten or more
  • Must be able to perform the essential functions of the job with reasonable or no accommodations.

Education

  • High school or equivalent

Required License or Certification

  • Driver's License required
